Premier Members should complete this rental agreement form and submit it to CAN^2®. You will receive notification of when the tape(s) will be shipped to you. If you have any questions, please call (630) 810-9029.

Select the video tape(s) you wish to rent for a maximum of 5 days. The rental period starts after a 2 day shipping allowance to you via UPS. At the end of the 5 days, use the original shipping container and the return label provided to return the tape(s) within 2 days to CAN2®. Any delays beyond this will result in additional fees. You will be charged $3.00 per day, per video tape, and the cost of shipping the tape(s) to you. You are responsible for the return shipping cost. After CAN2® receives this completed form, you will be notified of the shipping date. A credit card is required to process a video tape rental and to guarantee $75.00 per video tape in case of damage or loss. Your Shipping Address:
